I understand that I am being interviewed by Need-Hope.com Ministry (Ministry). I voluntarily agree to participate in this and future interviews, including being recorded (audio and/or video) and photographed.
I understand that all interviews will be used for Ministry purposes. I understand that my personal details, such as my phone number and address, will not be revealed to people outside of the interviews. I understand that all interviews, or portions of it, may be used in the Ministry’s publications, reports, web pages, podcasts, photos, and other media (Production).
I would like my real name used in the interviews. I understand that the interviews and any Production of it all belong to the Ministry and will remain the sole property of the Ministry, and the Ministry’s right to use it is perpetual.
I understand that the interviews are for the benefit of the Ministry and that I will not be compensated for them. I understand that any Production of the interviews could cause embarrassment or other harm, and I release Need-Hope.com Ministry and its agents, officers, directors, and representatives from any claims or liability that might arise out of or in connection with the interviews or the Production of it.
